Pharmacy Case Manager

About the position

LetsLink recruitment is currently seeking a highly motivated and experienced Pharmacy Case Manager to work for a private hospital in Johannesburg. As the Case Manager, you will be responsible for liaising between patients, treating clinicians, and medical aids to ensure accurate and comprehensive documentation of patients' hospital stays. Your role will also involve applying clinical knowledge to manage patient risk in terms of length of stay, level of care, and medical aid benefits.

Key Responsibilities:

• Apply clinical knowledge to manage patient risk
• Provide appropriate clinical information to medical aids
• Code all patients with ICD10 and CPT4 codes
• Accompany Managed Care Organisation Case Managers on ward rounds
• Assist with final authorization of patient files
• Control costs for patients
• Consult with all service suppliers regarding treatment and phone for patients
• Facilitate with transfer of patients to Rehab or step-down facilities
• Update all relevant details on Adclin Info
• Supply all details regarding patient's history, etc. to clinical partners
• Assist with case management of all medical aid patients
• Daily liaison and communication with Medical Schemes, Hospital Nursing staff, and medical practitioners

Requirements:

• Diploma or Degree in Nursing and registered with the SANC
• Grade 12/Matric
• Experience in ICD 10 and CPT 4 coding
• Intensive Care Nursing experience is an advantage
• Hospital Case Management is an advantage
• Minimum 3 years nursing experience in a multidisciplinary hospital environment
• Knowledge of Managed Healthcare policies and procedures
• General administrative skills
• Computer literate
