About the position
An established organisation is seeking a qualified Social Worker, Psychologist or Registered Counsellor to serve as the primary point of entry for community members seeking support. The successful candidate will have experience in intake, psychosocial assessments, crisis intervention, referrals and client support, with strong communication and confidentiality skills. The role requires sound judgement when assessing risk and identifying appropriate internal and external services.
Key Responsibilities
- Provide initial intake, emotional support, crisis intervention and containment.
- Assess psychosocial, financial, home-care and support needs.
- Conduct face-to-face, telephonic, home and hospital assessments.
- Assess and escalate high-risk matters appropriately.
- Guide clients through available services, admissions and documentation requirements.
- Refer clients to suitable internal, external and community services.
- Process, verify and maintain referral documentation and records.
- Build and maintain professional referral networks.
- Collaborate with families, professionals, community organisations and internal teams.
- Follow up, monitor and appropriately conclude or refer cases.
- Maintain strict confidentiality and comply with POPIA and professional requirements.
